Visa Required Data

The first form is a MVV visa application form, a declaration to undergo a TB-test, and an application fee collection form, including the following personal data:

  • Passport number
  • Signature
  • Optional: A residence permit given by another Schengen-member-country
  • Social security number
  • First name and last name
  • Gender
  • Date of birth
  • Place of birth
  • Country of birth
  • Nationality
  • Civil status
  • Home/Postal address
  • Telephone number
  • Email address
  • Name of the degree
  • Duration of the degree
  • The expected date of arrival in the Netherlands
  • Optional if Chinese citizen: A Nuffic-certificate

 

Attached to this form must be:

  • A copy of their passport

 

Antecedents Statement
The applicant needs to answer yes or no to the following questions. In case of a yes, an explanation is required:

  • Whether the student committed a crime with:
  • A prison sentence as a consequence;
  • Community service as a consequence;
  • An unconditional fine as a consequence;
  • A set fine as a consequence;
  • Whether the student received a penalty order due to committing a crime;
  • Whether the student is subject to prosecution due to committing a felony;
  • Whether the student has been responsible for an act as referred to in Article 1F of the 1951 Refugee Convention;
  • Whether the student has resided in the Netherlands illegally in the past;
  • Whether the student is subject to any entry bans.

 

Income statement
An applicant must convey which of the following financial options is applicable to his/her situation:

 

1. Self-finance

  • An original bank statement, including the information below, or;
  • An account statement, including:
  • First name and last name
  • The bank account number
  • The balance
  • Contact details of the bank

 

2. Third party support from inside the Netherlands
A statement of financial support, including:

  • The amount to be transferred monthly
  • Start and end date of the financial support

The student’s:

  • First name and last name
  • Date of birth
  • Place of birth
  • Nationality
  • Passport number

The financier’s

  • Copy of passport
  • First name and last name
  • Date of birth
  • Place of birth
  • Nationality
  • Passport number
  • Home address
  • Telephone number
  • Signature
  • Original extract from the Municipal Personal Records Database

In case the financier works in employment:

  • Copies of 3 months of salary statements, including:
  • Company name
  • Company address
  • First name and last name of the financier
  • Date of birth of the financier

The net salary amount

  • An original employer’s statement, including:
  • Company name
  • Company address
  • First name and last name of the financier
  • Date of birth of the financier
  • Type contract (temporary or permanent)

A copy of the employment contract, including:

  • Company name
  • Company address
  • First name and last name of the financier
  • Date of birth of the financier
  • Type contract (temporary or permanent)

In case the financier is self-employed:

  • Extract of the Chamber of Commerce register
  • Declaration on income
  • Copy of the provisional income tax assessment

 

3. Third party support from outside the Netherlands
A statement of financial support, including:

  • The amount to be transferred monthly
  • Start and end date of the financial support

The student’s:

The financier’s

The signatory’s:

5. Receiving a scholarship

A scholarship declaration, including:

  • The granting institution

  • The start and finish date of the scholarship

  • The amount of the scholarship

 

6. Deposit the required money in the account of RSM

A copy of RSM’s account statement, including:

  • First name and last name
  • Student number
  • Bank account number
